Request Your Records

Though our electronic health record doesn’t support a patient portal at this time, patients have a right to their records at any time.  For KCDPC patients who received care prior to 5/1/2022, you may reach out to KCDPC directly to request your records, or if you’d like them sent to a third party (like another doctor’s office), you can download this form, fill it out, and return it to us by email (preferred) or fax.  We will work as quickly as possible (barring tech snafus and vacations) to get your records sent; the maximum amount of time it should take is 30 days.
